By the end of Red Dragon, Lecter is directly responsible for ruining Graham's life twice - nearly causing the deaths of Graham's wife and stepson as well. Hannibal meant to kill him, but Mason survived to be a paraplegic with a grudge for the rest of his life. This allowed him to not only remember virtually anything he had learned, but to retreat to rooms within his mind whenever he was without his books or being tortured. Graham confronted his nemesis Lecter once more for help on the case, identifying the "Tooth Fairy" killer as Francis Dolarhyde, a film tech who was choosing targets through their home videos. Fredrick Chilton was never seen or heard from again. His sixth American victim, the bow hunter, was murdered and arranged in the style of the medieval drawing Wound Man, depicting many battle injuries. He attempted to drug and brainwash her into believing she really was Mischa so he could have a proxy of his sister back, but Clarice was too mentally strong, even for Lecter's considerable machinations, and she overpowered his attempts.

The mutilations he performs on his victims reflect his belief that chaos is the natural state of nature. He was instead sent to the Lecter, while in custody, was said to be "far too sophisticated" for most forms of psychological evaluation, especially as he enjoyed staying abreast of all of the latest developments in his field. Hannibal had also become mute from the traumas he'd survived — though he'd fully repressed the memory of eating his sister's flesh. Lady Murasaki visited him one last time while he was being held by the police, and saw that he had become completely emotionless. The FBI knew of 16 victims. Following his escape, Hannibal Lecter went through extensive plastic surgery on his face, only sparing his nose for fear he'd lose his extra-sensitive sense of smell. After his attack by Dolarhyde, Graham resented Lecter and spiraled into depression and alcoholism. Prior to Red Dragon, Graham had only met Lecter twice, both times very briefly, for Graham to ask Lecter routine questions; however, during their second meeting, Graham realized that Lecter was the killer he'd been seeking. Orphaned at a young age, Lecter moved to the United States of America, becoming a successful psychiatrist. He also started speaking again and added Japanese to his list of fluent languages. After Lecter is found living in Florence, Starling and Verger are desperate to catch Lecter. He first murdered a butcher who was rude to his aunt. All rights reserved He became world-renowned as a brilliant clinical psychiatrist, but he had nothing but disdain for psychology; he would later say he didn't consider it a science, criticizing it as "puerile", and comment that most psychology departments were filled with "ham radio enthusiasts and other personality-deficient buffs". While Hannibal might have been one of the most disturbing serial killers ever caught, he was also still one of the psychiatric community's greatest minds, especially when it came to people like himself.

Ultimately Lecter is rescued by Starling and Verger's sister Margot, Starling being injured during a shootout.
